BASICS

NAME. Your character's name.
OCCUPATION. Your character's occupation.
AGE/BIRTHDAY. Your character's age and birthday.

RELATIONSHIPS

FAMILY. Any family of importance who may or may not be making an appearance in-game. Other mentions of importance should be included under history.

NAME. About them if you'd like to go into detail.

SEXUALITY. Heterosexual, Homosexual, etc.
SIGNIFICANT OTHER. Girlfriend/Boyfriend, Fiance, or Spouse.
CHILDREN. Children and ages (if any).

APPEARANCE

What your character looks like, mannerisms, what others might assume seeing your character or hearing him or her speak, what are the first things other characters notice about your character, etc.

PERSONALITY

LIKES. Things they like.
DISLIKES. Things they dislike.

STRENGTHS. Things they're boss at.
WEAKNESSES. Things they suck at.

GREATEST FEAR. Greatest fears (think outside the box and try to pick less obvious ones like family deaths).

DETAILED PERSONALITY.

Your character's personality. Try to aim for at least three solid paragraphs. Particularly try to hit on their driving motivations and opportunities for development during the game.

This is where you establish the core values and traits that make up your characters and are at the base of their actions. This helps to keep your character's personality anchored to something while you're writing (that you can use as a reference while playing). It prevents A) you from totally derailing from the personality you've established or B) making one or two character traits/quirks the character's only defining characteristic. Also, because your character's core personality is established, you're free to focus more on opportunities for them to develop during the game.

If you're having trouble getting started, a few ideas for things to talk about include: what sort of person other characters believe yours is, your character's opinion of him/herself, your character's ambitions, how your character handles bad situations, habits, frustrations, your character's reputation, moral dilemmas your character may face, what your character admires/dislikes in other people, when your character feels most/least at ease, your character's priorities/ life philosophy, quirks, what your character wants most out of life, past failure s/he would be embarrassed to have people know about, vulnerabilities -- perhaps how they hide it, their greatest assets, etc.

HISTORY

A history of your character. This shouldn't read like a grocery list. Rather, it should take into account how your take on this character's personality/history affected who he or she became. So, in a sense, this is your way of selling your interpretation.

As said in PERSONALITY, make sure to show opportunities for development during the game.

If you need suggestions, areas you can hit on include what sort of home/class your character comes from, mentions of parents and siblings and how they affected your character, the sort of friends your character made (or didn't make) and why, what clubs and affiliations they might have belonged to, hobbies, what sort of student they were, your character's occupation(s) and what caused them to go into the line of work they are now in, goals/ambitions, etc. Still need ideas? There's a list of 365 character questions here.

FIVE THINGS

Five things you'd like to RP out/explore about this character or alternatively why you picked this character, things you're most looking forward to playing out with this character, etc. Try to include at least one plot idea for your character. Providing this information helps the mod and other players build plots around your character.
