Normally, Kelly isn't easy to sneak up on. Right now, she's barely able to hear anything for the sound of machinegun fire echoing in her ears. And a little out of sorts, because she currently contains half her body weight in lead, thanks to the goons she's been dispatching, making the job somewhat easier.
More to the point, Weapon X mostly uses her as a sniper, and doesn't let her have this much /fun/. So she's not paying a ton of attention directly behind her, thinking she had accounted for everyone - other than Bea's intuition telling her just when to duck into the ladies room during Kelly's recon.
