All the main AC boys, Desmond, Altair and Ezio, are modeled after Francisco Randez~ Altair and Desmond are really close to his appearance, though in AC2 they did seem to give Desmond a bit of a rounder face.

Ezio doesn't look nearly as close to his face as Desmond or Altair, as he's a lot rounder in the face and his cheeks aren't as defined and the long hair and... he's Ezio? XDDD

BUT YEAH. They picked a sexy man to model them after.
